Records required by the Ed. Code (and so required by Liberty Preparatory Academy)

 

 

  1. Attendance records must be kept in a register. I require 165 days (public schools use at least 174). This attendance record will be on a form which is available to you by download or mail. What is required by the Academy is a simple monthly email with dates of attendance listed. These will be transferred onto the form kept at the school. Codes used are p = present, a = absent, / = wknd, h = holiday, v = vacation.
  2. A Course of Study is a list of books (curriculum) and publishers/authors and grade level, for all subjects this child is taking this year. This is what you “plan to use”. This can be done on the downloadable form or emailed to the school.
  3. The ed code says teachers in private schools “must be capable of teaching”. This list of teacher qualifications can include classes you’ve taught, workshops and conventions attended, Sunday School classes taught, scouts, 4-H, FFA, education, etc. Don’t forget your husband’s qualifications!

 

 

The cumulative file belongs to the student. It is a student’s record of classes attempted, classes passed including grades, workshops attended. Immunization record and/or waiver needs to be in this file. Either the “blue card” from the county you are in, or a downloadable form need to be filled out and returned to the Academy by mail. A copy of the "yellow" immunization record that the doctor gives will be acceptable. The “course of study” for the current year goes in here until replaced by a report card. As for report cards…you are the person primarily in charge of your child’s education, so it is up to you. For elementary level grades are not important. For jr. high and high school they are necessary if the child plans on college. The Academy will provide a report card twice a year, unless needed more often, as you provide me with the child’s grades. The school will also provide transcripts for high school students.

I.D. cards will be provided for students and teachers…. either email me a photo, or I will take a photo when I see you (for those in closer proximity).

The Academy will also request cumulative files from previous school if necessary.
